Nephrolithotomy may be a good alternative when the stone is about 4/5 of an inch in diameter which is exactly the size of a nickel or bigger. It is also done in cases when the stones are very dense or the patient has branched stones, a condition called Staghorn that blocks a large part of the kidneys.

The PCNL procedure usually takes one to four hours depending upon the size of the stone, the number, and its location.

It takes around a week to two weeks to recover fully from the procedure.
